Fill in the required information and submit the request electronically.
Eligibility Review: The applicant's eligibility is verified for the following categories:
Persons with disabilities
Elderly (over 60 years old)
Home healthcare patients
Cases approved by the medical committee
Wait for approval from the Patient Experience Department or the Awlawiya Office electronically.
Receive the card electronically without the need to visit the healthcare facility.

People with disabilities
Older people over 60 years old
Home health care patients
Cases approved by the medical committee
Those who have medical files in the health facility
Beneficiaries of health services in health facilities affiliated to the Ministry of Health
